چکیده
M e t h o d s Design: Randomized controlled trial (Second Randomized Intervention Treatment of Angina [RITA-2] Trial). Allocation: {Concealed}†.* Blinding: Blinded (outcome assessors).* Follow-up period: Median 7 years. Setting: 20 centers in the United Kingdom and Ireland. Patients: 1018 patients > 18 years of age (median age 58 y, 82% men) with arteriographically proven coronary artery disease who were considered suitable for either PTCA or medical therapy. Exclusion criteria were previous myocardial revascularization, left main stem disease, recent acute coronary syndrome, hemodynamically significant valve disease, life-threatening noncardiac disease, or need for revascularization. Intervention: Patients were allocated to PTCA (n = 504) or conservative medical therapy consisting of appropriate antiangina medication for symptom relief (n = 514). Outcomes: Composite endpoint of death or definite nonfatal myocardial infarction (MI). Secondary outcomes included all-cause mortality, MI, cardiac death, and angina. Patient follow-up: 98% (intention-to-treat analysis).

The evaluation of a patient with a history suggestive of coronary artery disease is familiar to any internist. After a careful history and physical examination, the internist often turns to exercise testing to provide further diagnostic information. If the patient has an abnormal test result, the physician is faced with deciding whether to refer the patient for more invasive testing and therapy...

The 3 to 5-year clinical outcomes of 627 consecutive patients selected for treatment with percutaneous transluminal coronary angioplasty (PTCA) at Emory University Hospital for one- or two-vessel coronary artery disease with involvement of the left anterior descending coronary artery, between July 1981 and June 1983, and 865 patients in the Coronary Artery Surgery Study with similar coronary ar...

In the past, coronary artery bypass grafting was the treatment of choice for patients with symptomatic multi vessel coronary artery disease, but in recent years per cutaneous transluminal coronary angioplasty (PTCA) accepted as an alternative approach to revascularization. To assess the initial success and in hospital results of coronary angioplasty of more than one lesion per procedure in pati...
متن کاملIs a US analysis of cost-effectiveness in interventional cardiology relevant to a centrally funded health care system?
Conclusions In today's health care environment, decisions about medical interventions need to reflect measures of cost as well as clinical benefit. While it is clear that PTCA is significantly more expensive than medical therapy alone, analyses incorporating quality of life considerations suggest that angioplasty techniques that have been shown to improve clinical outcomes are, for the most par...
